I went through the middle-groundist phase during my GC conversion process, and their position may well change as did mine into a hard-no. While there is a tiny cohort of people who do authentically believe themselves to be the opposite sex these people are swamped by a much larger population of those just using that narrative because they learned it gets them whatever they want.
When studying psychology I remember being taught that this is a 1/10000 phenomenon. Medicine is meant to be treating people's issues not enabling them (at others' cost!), in this case cost of half the population. Society is sufficiently tolerant of being gay today there is no longer that excuse of having to conceal being male by "gender reassignment".

It clearly establishes that the motivation for men to "present as women" is sexual rather than some fictional gender identity.
