That first part is a single sentence so "To the extent possible" means where it is possible to provide alternative accommodation to the kid the school should to so but where not possible the kid should just be excluded.
The second part deals with as you said the scenario where the law of the state requires schools to allow these kids access to opposite sex spaces (so the school has no choice) then all other parents should be told of the situation.
Remember this is an attempt at guidance it is not a set of rules. Setting rules is a job for school governors and the DoE.
